At a certain point in the postoperative trajectory, patients were not given morphine regularly and were expected to request the drug if they felt they needed it. Seven of the 10 children were on IV morphine infusions prescribed on sliding scales. The findings indicate that there was a reluctance to increase the rate. The rate was only observed being increased twice (Cases 5 and 9). Medications were rarely administered for breakthrough pain. Nurses did not always give children additional doses of pain medi- cations even if their pain was 5 or greater. This was despite analgesics usually being prescribed for breakthrough pain.
