● 0 = Incidents neither reported nor investigated.
● 1 = Incidents are reported only.
● 2 = Incidents are reported and investigated to determine root causes (suitable root cause analysis tool employed) there are some evidence of measuring, tracking, and utilizing a safety incident rate, but is incomplete and/or not well documented.
● 3 = Incidents are reported and investigated in a reasonable time to determine root causes (suitable root cause analysis tool employed) and corrective actions are planned and implemented, evidence of measuring (e.g. incidents / 100 employees), tracking, and utilizing a safety incident rate, is available, documented, and posted for all employees to see.
● 4 = All of level 3 plus, unsafe actions or conditions (Near Misses) are identified, reported, investigated and corrective or preventive actions are taken, evidence of a Year-Over-Year reduction in the safety incident rate is demonstrated."
