1. An employee must register as the insured person within 30 days from the date of employment.
2. Filling the insured registration form (SorPhoSor.1-03) in the case where an employee has never been registered in the social security system before.
3. A copy of ID Card or other cards issued by governmental agencies
4. A copy of Work Permit and copy of passport or a copy of Work Permit and copy of Certificate of Alien in the case where the insured is a non-resident
5. An employee, who has been registered as the insured person before, must inform the former registration (SorPhoSor.1-03/1)
** ln case of individual owner, the business owner is an employer so he/she can not register as an insured person **
