The patient’s hemoglobin level was 12.9 g/dL, and the hematocrit was 39.2%. White blood cell and platelet counts were normal. Results of a basic metabolic panel revealed an elevated creatinine level of 1.91 mg/dL (to convert to micromoles per liter, multiply by 88.4), with an unknown baseline value. Hepatic function tests, coagulation assays, erythrocyte sedimentation rate, level of C-reactive protein, level of haptoglobin, and results of urinalysis were normal. The initial fecal occult blood test was negative for blood. High-resolution chest CT at the time of admission showed multiple, bilateral, noncalcified, subcentimeter pulmonary nodules (Figure 1). Two-dimensional echocardiography showed a normal ejection fraction and mild aortic stenosis.
