We used a Cox proportional hazards model with time since return of the first post-MI food frequency questionnaire as the underlying time scale. Person months were calculated from the date of the first food frequency questionnaire returned after initial MI until death or the end of study period, whichever came first. We also evaluated the association between pre-MI fiber intake with post-MI survival starting at the date of initial onset of MI. The proportional hazards assumption was tested by modeling multiplicative interaction terms between time and the primary exposure variable. For analyses of trends, we fit medians of each fifth as a continuous variable.
