7 CONCLUSION
The cracks observed on the stationary platen of the plastic injection moulding machine
(new machine #2) at plant 2 of Toyota Boshuku Gateway was likely to be a result of the
initiation and propagation of a fatigue crack. There were no unusual events reported prior
to the discovery of the cracks and they were discovered during a routine inspection of
the machine.
In a year, the modified moulds were used so infrequently on new machine #2. It was difficult
to appreciate how the modified moulds could contribute to the crack on the machine.
Most of the time, the machine was operated with its original moulds. The crack
observed on the machine was likely to have occurred as a result of normal operation.
