The following are some basic guidelines that might help an information manager gather information about users by questionnaire:
o keep the questionnaire short and simple
o try to put the questions in a logical order
o use simple words, plain and easy to understand language
o keep sentences short, and avoid ambiguity
o use a clear type face that is easy to read and understand
o pay attention to the layout and design, and
o decide what exactly it is you want to know, formulate the question, then ask yourself whether you would give an answer to the question that would be appropriate for the research; modify, if necessary.
