requests were made on several English-speaking
listservs throughout the world (e.g., the United
Kingdom, Canada, and Australia). The survey took
approximately 10 minutes to complete. In addition
to demographic information, educational background,
and the nature of their clinical practice,
respondents were asked about their clinical experiences
in those areas specified above. Specifically, they
