The most accurate method is direct observed therapy.
This is the long, intensive process that is used in hospitals
to watch the patient take the medicine. It is not practical
for routine use among outpatients. Biomarkers for assessing
drug levels are not always available and don’t account
for variations in metabolism; also, the assays are expensive.
Electronic monitors such as those used for MEMS are
very costly.
