1. Explain the procedure to the patient and gain consent.

2. Ideally sit patient on the edge of the bed or if not appropriate, lay patient on their side.
To try and ensure full access to nephrostomy site.
3. Wash hands and put on apron.

4. Open sterile pack and prepare sterile field and equipment.

5. Apply sterile gloves. Gently peel away old pouch using non-alcohol adhesive remover if appropriate, or supporting the skin with your other hand to prevent skin damage or pulling the tube.
