B. Review of Historical Databases
Another source of reliability data is the review of historical
databases of electrical utilities. The analysis of a sufficiently
large sample of events that involved operator response would
help establish a database indicating the frequency of operator
errors ( ) and the time required to form sufficient SA under
different system conditions ( ). However, existing databases
are limited to incidents that involved actual loss of load. Much
better data could be extracted from these databases if they also
included information about near misses.
