Institute measures to prevent the development of pressure ulcers if the patient is at risk for this condition. Institute measures to treat redness or open areas if these conditions are revealed during the skin assessment.
Provide skin and perineal care frequently to a patient who is incontinent of urine or stool to prevent IAD and pressure ulcers.
Protect the patient from injury by assessing and controlling the bathwater temperature. This is especially important for older adult patients, patients with reduced sensation, and those who cannot communicate.
