Observed one operator use safety harness with 6 ft. lanyard for ascend to top 90 bbls. fluid tank. He need to R/U temporary handrail on top tank. But top tank was high from floor only 4 ft. It did help him when he fall from tank. He told he did not know length of lanyard and do not check it before use. He just wear and think safety equipment ready to all the time. He told explain the solution in this case. He must check safety equipment before use and ensure lanyard is match with the high of scope of work. He suggest to review all equipment which must required to work on top and measure height. Eliminate long lanyard out of toolbox.

While crane and roustabout loading food cargo into sack room, observed two men were standing on red zone, under load. Potential to get incident and drop objects. SWA used, told them move away from that area, explained risk and hazard.

Unsafe Act or Unsafe Condition: Observed crew of supply boat hurry to unhook when load not placed on the floor of supply boat yet. SWA, explained hazard and told them to follow signals

Found air leaking from koomey unit, it could be a problem with well control. I Informed Toolpusher and Mechanic to check.

Found TBRA is not up to date and not the same as the operation which might be confused to crew why the TBRA and operation is different on the rig floor. Review TBRA, revised and updated. Make sure to bring TBRA to Toolpusher to confirm.
