Representative Case
A 50-year-old man with history of hypertension devel- oped sudden-onset headache followed by left-sided hemi- paresis and depressed level of consciousness during the next 2 hours. On arrival to the emergency department, he had initial somnolence with GCS score of 11. Head CT revealed a 20-mL right basal ganglia hematoma without midline shift. Therefore, conservative treatment and close surveillance were performed. However, 36 hours later he became semicomatose and a detailed examination showed his GCS score of 7 with right mydriasis. Head CT revealed the hematoma increasing to 65mL with prominent midline shift. Decompressive craniectomy in addition to hematoma evacuation was performed successfully 38 hours after the ictus.
