Two initial screening questions.
1. Over the past 2 week, have you felt down, depressed, or hopeless?
2. Over the past 2 week, have you felt little interest or pleasure in doing things?
If one or both questions are positive
Evaluate major depression based on the following DSM-IV criteria.
Five (or more) of the following nine symptoms have been present during the same 2-week period and represent a change from previous functioning; at least one symptom is either depressed mood or loss of interest or pleasure.
1.Depressed mood.
2.Loss of interest or pleasure.
3.Significant weight loss (when not dieting) or weight gain or decrease in appetite.
4.Insomnia or hypersomnia.
5.Psychomotor agitation or retardation.
6. Fatigue or loss of energy.
7. Feeling of worthlessness or inappropriate and/or excessive guilt.
8. Diminished ability to think or concentrate, or indecisivenss.
9. Recurrent thoughts of death or suicide.
