Form must be registered on-line, printed out and signed by the applicant.
All entries should be properly filled out - no blanks.
• 1 passport size photograph

• original passport
Must be valid at least 3 months calculating from the date of intended departure from the Schengen area.
Must contain at least two blank visa pages, excluding the endorsements pages.
• copy of the passport
• copy of ID card
• work permit
Original document issued by relevant Polish local authorities must be presented.
Work permit presented at the Embassy shall not be returned as it is archieved with the full documentation of the applicant. There is an additional original issued to the Employer by the Polish Authorities for whatever purpose it may serve.
• employer's written declaration
Only in case the work permit is not required according to the Polish regulations (does not apply to Thai, Lao, Cambodian and Myanmese nationals). The employer's letter must confirm the intention to employ the alien as well as place and character of employment.
• health insurance
Insurance certificate must cover the travelling period as well as the time until the actual start of performing duties (eg. indicated by the agreement). Insurance certificate issued by the Polish insurance company shall be accepted. The 30,000 Euro coverage rule does not apply.
• confirmed flight ticket reservation
Round-trip booking rule does not apply. One-way reservations shall be accepted.
*IMPORTANT NOTE:
Visas for the purpose of carrying out work are issued only by the Polish Consul at the office relevant to the applicant's legal permanent residence / country of origin. This is a Schengen or national type of visa issued for the period of stay indicated in the work permit but not exceeding one year. Employment without prior authorization and relevant visa is strictly prohibited.
