8.1 Training records
1. The training must be documented and include performance 'Objective Based Training', testing for successful completion of the program. Each associate who participates in the fall protection/arrest program training must demonstrate competency in the use of fall protection/arrest equipment.
2. The written certification records must contain
1. the name of the trainer,
2. the date(s) of the training,
3. course outline, and
4. the signature of the associate trained.
3. If the training is conducted by another employer or prior training conducted outside the facility, then the certification record must indicate the date that facility determined the prior training to be adequate, rather than the date of actual training.
8.2 Certification: Each associate must be certified as having received the training and must demonstrate competency in the use of fall protection/arrest equipment. Training of affected personnel must be document training.
8.3 Job re-certification must be conducted to determine if the associate has retained the necessary knowledge and competence to effectively use fall protection/arrest equipment.
8.4 Refresher Training: The employer must retrain the associate when there is reason to believe that the associate has been unable to demonstrate the skill and understanding required with reference to 'Fall Protection'.
