For each statement a student receives a score of 1, 2, or 3. For most items when the
“Yes” is circled, this counts as a 3 and a circled “No” response is worth 1. Statements
that are left blank or are otherwise invalid (such as when both choices are circled)
receive a score of 2. Some statements have their number underlined, and include an R
in the teacher column. This indicates the wording of the statement is reversed and the
scoring is reversed. For only those items a “Yes” answer is worth 1 and a “No” answer
is worth 3. Using this procedure you can establish a score for each statement. The next
step is to find the score for each subscale.
