Questionnaire
This questionnaire describes a number of common experiences most of you have in your daily lives.
These statements are presented one at a time and following each are two possible answers. Read the description of the experience carefully and then look at the two answers. Choose the one that most often describes what happens to you. Put a circle around the A or B in front of that answer. Be sure to answer each question according to how you really feel.
If at any time you are uncertain about the meaning of a question raise your hand and one of the persons who passed out the questionnaires will come and explain it to you.
