Team Member fills out the Conflict of Interest Disclosure Form
2. Team Member sends the Conflict of Interest Disclosure Form to their direct manager.
3. For consulting contracts with a projected annual cumulative value of $1000 CAD or less, the
manager may decide (at their discretion) that a Conflict of Interest situation does not require
reporting. In the Field Offices and Regional locations in developing countries, this value is $250 CAD.
4. The form is presented to the Ethics Committee at Headquarters for evaluation, and the Team
Member may be contacted to request further information, if deemed necessary.
