The author has experienced 96 fresh cases of hypopharyngeal cancer at Kinki University. In 36 T3/T4 cases, laryngectomy. partial pharyngeal membrane were conducted as mentioned above. Fifty-eight T1/T2 cases underwent laser debulking and irradiation to preserve the laryngopharyngo-esophagectomy and a free jejunum autograft. In these two cases, the tumor almost completely occupied the hypopharynx in all directions, and it was thought to be impossible to maintain a normal mucosa. Currently, when T1/T2 cases account for 60% advanced T3/T4 cases, for wich a normal mucosa membran of the hypopharynx of more than 3 cm in width can not be maintained, account for less than 10%. This is one reason why recent reports about partial excision of the hypopharynx preserving the larynx are increasing.
