We found a lot of ferrules leak/damage after choke valve tubing reinstatement.

We have been informed that many joints of hydraulic tubing got leaked during choke valve function test (more than 70 joints). After investigated we found many tube’s ferrules and nuts (tube’s fitting) got damaged caused from improper and over tighten during final assembly by Hydraulic Oil Flushing team. Could you please ask the responsibility from vendor who performed the Hydraulic Oil Flushing.
Please be informed that these tubes were accepted the final installation check and witness the tubing leak test by MP inspector before handover to perform the hydraulic oil flushing.

1. In case the tube’s fitting (ferrule & nut) damaged and require to change, we need to change related tube also because we need to cut tube for remove ferrule & nut.
2. Who will absorb material & manpower cost for the above item 1
3. How to ensure that this issue will not occur on another hydraulic tube packages which related to hydraulic oil flushing activity.
4. Kindly consider to WHP-B also, do we need to spare material.
