A variety of predictors of early hospital readmission
after discharge for patients with congestive
heart failure (CHF) are described in the current
literature. Positive predictors are the patients’
comorbidities as shown in the Charlson Comorbidity
Index,5 noncompliance with medication, diet, and
exercise,6-12 posthospital medication discrepancies
defined as “the assessment focused on what older
patients reported taking in comparison with the prehospital
medication regimen and the posthospital
medication regimen,”13 lack of cardiology consult during admission, living alone, receiving Medicare,
and pulmonary hypertension
