Before embarking on any proposal to introduce a new concept
such as a nurse-led clinic, it is beneficial to identify potential
barriers to effective change. Lewin's (1951) force-field analysis is a
useful model to review these issues {Table 3) and this was utilized
in preparing the proposal to present to the directorate manager
and rehabilitation consultant to gain approval for the project.
Research cites a number of threats to the development and
success of nurse-led initiatives, ranging fi^om a lack of fijnding,
poor planning, doctors' reluctance to relinquish aspects of their
role (Cable, 1995) and, in some instances, resistance to the new
working practices may be experienced. An additional and realistic
risk is associated with ineffectiveness in care, resulting in patient
complaints and litigation (Miles et al, 2003).This highlights the
importance of nurses working within the boundaries of their
professional knowledge and competence in line with the Code of
Professional Conduct (Nursing and Midwifery Council (NMC),
2002). It was discussed and agreed by the specialist nurses to
coordinate the six clinics together, thus providing ongoing
support and professional development. Richardson and Cunliffe
(2003) advocate supervised practice to prevent isolation.
