A 40 year old female patient had mite bite on her left hand 10 days back.. now she came to our surgery department with cellulitis of left hand & mild anasarca... fasciotomy was done... she is getting continuous fever.. her all investigations were with in the normal limits initially... except for raised ESR..
from the 13th day of mite bite there was gradual decrease in platelet count & noticed pleural effusion.. inspite of treating with Antibiotics ( cephalosporins & piperacillin-tazobactam ) she is getting fever... she was disoriented, & BP also fall to 90/60
what could be the probable diagnosis ??

จากวันที่ 13 ไรกัดมีการลดลงอย่างค่อยเป็นค่อยไปในเกล็ดเลือดนับและสังเกตเห็นปอดไหล .. แม้การรักษาด้วยยาปฏิชีวนะ (cephalosporins และ piperacillin-tazobactam) เธอจะได้รับไข้ ... เธอถูก disoriented และ BP ยังตกอยู่ที่ 90/60
สิ่งที่อาจจะวินิจฉัยได้ค่อนข้างแน่ ??

ผู้ป่วยหญิงอายุ 40 ปี มีไรกัดมือซ้ายของเธอ 10 วันกลับ . . . . . . . ตอนนี้เธอมาถึงแผนกผ่าตัดของเรากับโรคผิวหนังของมือซ้าย&ผักกะเฉดอ่อน . . . . . . . ดัดผมเสร็จ . . . . . . . เธอจะ . . . . . . . ไข้แบบต่อเนื่อง เธอสืบสวนอยู่ในขอบเขตปกติในตอนแรก . . . . . . . ยกเว้นจาก . . . . . . .
ตั้งขึ้นจากวันที่ 13 ไรกัดก็ค่อยๆลดลงใน&เกล็ดเลือดสังเกตห้าง . . . . . . . แม้ว่าการรักษาด้วยยาปฏิชีวนะ ( ยา&ปิปเปอราซิลลินทาโซแบคแตม ) เธอจะไข้ขึ้น . . . เธอกำลังสับสน & BP ยังตก 90 / 60
