If your sim has one of these traits:

Loser
Evil
Mean Spirited
Unlucky (possibly)

Then that's why. This is an easy fix. Open up your cheat console by pressing shift+ctrl+C. Then type in the cheat console "testingcheatsenabled true" just like that but without quotes. Then shifft+left click on your sim, then choose the option "Modify Traits". The trait selection screen pops up and you can re-choose your sim's traits. Just simply replace the ones that are stopping your sim from being taken by the grim reaper.. Then kill your sim again. The Grim Reaper should then take your sim. Hope this helps. :D
