These questionnaires are intended to understand legal functions (including relationships with lawyers) in each group company, especially in overseas group companies and Global Management Platform so that we can study how our legal support should be.

Please kindly answer the questionnaire listed below and fill in the answer in the response sheet hereto attached (hereinafter “Sheet”). If you do not know the exact number as to Question Nos. 2(1), (3), (4) and (5), please answer by estimate.
We appreciate your cooperation in advance.

1. Questions on Corporate General Information
