The advance practice nurse and pharmacist asked directed question about medication taking behavior and adherence. At each visit, the dietician assessed the patient's nutritional and weight management status. In addition, the dietician reviewed any recommended dietary restiction, proided suggestions for food alternatives, and counseled on weight management strategies when appropriate. The social worker also assessed the patient during the clinic visit reviewing advanced care directives, financial, or social support issues identified during the interection. The social worker assessed the patient's overall coping response to his or her chronic kidney disease and inquired about any major life changes.
