First of all I would like to apologize for what happening. Follow up from the meeting the below is action item from us will be done immediately.
- Recall all product of Flow coupling and X-nipple which still in Baker to re-inspect and provide final inspection for each item and do proper report for the defect item in case it is found during re inspection.
- Result of investigation , Root cause analysis and corrective action will be prepared with the proper report of incident within 2 week time.
Moreover , OMS looking forward to improve QA/QC process to eliminate the Gap of quality issue which the detail of improvement will be presented to QA/QC Chevron and discussed with them to seek the opportunity for enhancement in the long term way.

Please contact with Material coordinator from Baker to recall back holes lot for what we send to them. Please re inspect all pieces carefully , provide the proper documentation and put this to your first priority. We need to resend it back to Client as soon as possible.
