Points to note when filling out the form:
• Immediate supervisor is to fill out the form and the Head of the Department is to counter-sign before submitting to Human Resources by the deadline indicated above.
• If the staff’s confirmation date falls within one month of the year end review process, a performance review will not be necessary for that year.
• In accordance with ACE policy, there is no salary adjustment upon confirmation.
• The performance of the staff should have been monitored from the first day of employment. Verbal communication of any ‘below expectation’ performance should have been communicated to the employee during the probationary period.
• If current performance is deemed to be ‘below expectations’, the manager can choose to continue the probationary period or may end the employment.
