3. Discussion
There arefewreportsof A.terreus colonisation oftheCFair-
ways,allofwhicharefromadultpatients [3,4]. A.terreus emerged
as adominantandpersistentmemberofthepolymicrobialcom-
munity presentinthischild'sairwayfollowinglong-termco-co-
lonisation with P.aeruginosa and S. aureus and aprolongedperiod
of exposuretohigh-doseantibiotics(Supplementary material).
Exposure tohigh-doseantibioticsisapredisposingfactorforde-
velopment offungalinfectionsandinparticularhasbeenlinkedto
the developmentofaspergillosisinCF [16]. PeoplewithCFhavea
high exposuretoantibioticsovertheirlifetimeandassuchare
predisposed todevelopingfungalinfections.
A significant declineinlungfunctioncoincidedwithhighanti-
A.fumigatus IgG levelsof44.1mg/L(abovethe40mg/Lthreshold
[12]), hightotalIgG,totalIgAandhighanti-A.fumigatus IgE levels
but notwith A.fumigatus or A.terreus culture positivity.Thirteen
months afterdetectionofanti-A.fumigatus IgG, A.fumigatus was
detectedinthepatientssputumhoweverthiswassporadicandno
A.fumigatus wasculturedfromthefollowing19sputumsamples.
Fourteenmonthsafterdetectionofanti-A.fumigatus IgG, A.terreus
wasdetectedfromthepatientssputumandwasconsistently
present inthepatientsairwaysthereon.Ithasbeenpreviously
reportedthatserumantibodylevelscanbeelevatedinadvanceof
sputum culturepositivityfor Aspergillus species [17]. Monitoring
anti-Aspergillus IgG andIgElevelsinvulnerablepatientssuchasCF
patients couldpickupearlycolonisationbeforeculturepositivity.
In thecasepresentedhereculturemethodswereusedtodetect
microorganisms howevermoresensitivemolecularmethodsmay
havedetected A.terreus colonisation earlierormayhavedetected
the presenceof A.fumigatus in moresamples. A.fumigatus was
only culturedon4occasionsfromthispatient'ssputawhereasthe
patient waschronicallycolonisedwith A.terreus following initial
isolation. Despitethesporadicandlimitedisolationof A.fumigatus
from thispatient'ssputa,high A.fumigatus-specific IgGandIgE
antibodies weredetectedinthepatient'sserum.Therearenore-
ports intheliteratureofcross-reactivitybetween A.fumigatus and
A.terreus allergens usingthePhadiaImmunoCapM3assay.Pha-
dia's onlineinformationspecifies that “A.terreus, A. flavusandA.
nidulans hydrolysecollagenandwerefoundtosecreteanalkaline
protease relatedtothatofA.fumigatus”. Althoughtheallergenic
potentialofthisalkalineproteasehasnotbeendeterminedit
could besimilartootherallergenicalkalineproteasesfromother
fungal speciesandcausepotentialcross-reactivitywith A.terreus
using theImmunoCapM3assay. A.terreus has alsobeenshownto
produce allergenshomologoustoAspf2(UniProt:
Q0CZU2_ASPTN), Aspf4(UniProt:Q0CB03_ASPTN,
Q0CTT7_ASPTN, Q0C97_ASPTN), Aspf7(UniProt:
Q0CL73_ASPTN)andAspf15(UniProt:Q0CYJ2_ASPTN)which
could causepotentialcross-reactivityastheseareallidentified A.
fumigatus allergens. The potentialforcross-reactivitywithinthe
Aspergilli genus usingtheImmunoCapM3assaywarrantsfur-
ther exploration.
Lung functiondidnotimproveandradiologicalappearances
worsened overa4yearperioddespitetreatmentwithnumerous
antibiotics andthreeazoles.Followingcompletecollapseofthe
right lungthedecisionwastakentoperformarightlungpneu-
monectomy.Post-pneumonectomylungfunctionreturnedto
baseline levelshoweverthepatientcontinuedtoculture A.terreus
and anti-Aspergillus IgG levelscontinuedtorise,withamostre-
cent levelof74.2mg/Lrecordedinmonth66.Baxter et al. [12]
demonstratedthatan Aspergillus IgG levelofmorethan75mg/L
can identifypatientswithABPAserologyorbronchitiswith90%
sensitivityand96%specificity.Baxterandcolleaguessuggestthat
“the useofIgG,usingacutoffof75mg/L,couldbecomemoreclini-
cally usefulinpracticeforinvestigatingandmonitoringpatients

clinically suspectedtohaveAspergillusdisease”. AstudybyBarton
et al. [17] suggestedacutoffof90mg/Lforseparatingpatients
with ABPAfromthosewith Aspergillus sensitisation orcontrols.
The patientrepresentedinthiscasereporthadanti-Aspergillus IgG
levelsclosetothe75mg/Lthresholdpost-pneumonectomyand
with thetrendofincreasinganti-Aspergillus IgG levelsobserved
overthepast4yearsandcontinuedcultureof A.terreus from
airwaysamples,thepatientsanti-Aspergillus IgG levelsshouldbe
monitoredclosely.
Despite in vitro sensitivity of A.terreus isolates totheazoles,
therapyfailedtoeradicate A.terreus from thepatient'sairways.It
is alsoworthnotingthatthereweredifficulties inmaintaining
therapeuticlevelsoftheazoledrugsinthepatient'ssera.Inthis
report later A.terreus isolates, post-voriconazoletreatment,tested
intermediateforresistancetovoriconazolehavingbeensensitive
prior tothis(TableS2). HereweusedECVsastherearenostan-
dardisedbreakpointsforantifungaldrugresistancepublished.
ECVs representtheMICvalueidentifyingtheupperlimitofthe
wild typepopulationandhavebeensuggestedtohelpcharacterise
the susceptibilityofisolatestoantifungals [13]. Ofnote,these
isolates werealsoindependentlydeterminedasintermediately
resistanttovoriconazolebytheHealthProtectionAgencyMycol-
ogy ReferenceLaboratoryinBristol,UK(datanotshown). A.terreus
clinical isolatesresistanttothetriazoleshavepreviouslybeen
reported [8,10]. These findings highlightthe need forroutine
antifungal drugresistancetestingandinparticular,monitoring
of patientsthatreceiveregularhigh-doseazoletherapysuchas
CF patients.
During thisstudy,atypicalisolatesof A.terreus weremis-
identified as P.variotii. Previously, A.terreus isolates fromthis
patient hadgrownwithtypicalcinnamonbrownconidialmasses
and theisolatesmis-identified as P.variotii had yellowconidial
masses andsomeclonalvariantsgrewonlywhitehyphalmasses.
Microscopicallytheseisolateswerealsoatypicalof A.terreus
havingcollapsedvesicleswithreducednumbersofmetulae
(eliminating thecircularringofmedullatypicalofthespecies)and
few phialidesgivingrisetosparse,shortchainsofconidia.Mi-
croscopicallytheseisolatesappearedsimilarto Penicillium how-
ever theiryellowconidialedtotheirspeciationas P.variotii. Dueto
this suddenappearanceof P.variotii, afungalspeciesrarelyiso-
lated fromCFairwaysamples,wesoughtconfirmatory macro-and
microscopicidentification fromthefungalreferencelaboratoryin
Bristol,UKandwealsosequencedtheITS, β-tubulin andcalmo-
dulin regionsfromtheatypicalisolates.Alsoantifungalsensitivity
testingwasperformedas P.variotii is susceptibletoAmphotericin
B [18] while A.terreus is intrinsicallyresistanttoAmphotericinB.
The isolatesweresubsequentlyconfirmed as A.terreus. Itisin-
terestingthattheatypicalgrowthof A.terreus emergedwhenthe
patient switchedfromvoriconazoletoitraconazoletherapy.Aty-
pical growthof Aspergillus exposedtoazoleshasbeenpreviously
reportedtointerferewithfungalidentification [19]. A. terreus and
P.variotii are usuallyeasilydistinguishablebymacro-and
microscopicmethodshowevertheatypicalgrowthofthe A.
terreus isolates, perhapsduetoinfluences ofazoletherapy,
resultedinmis-identification. A patient'sexposuretoantifungals
should beconsideredwhenusingmorphologicalcharacteristicsto
identify colonisingfungalspecies.
The A. terreus isolates collectedinthisstudywerepatho-
genicinthe G. mellonella infection model. Galleria arein-
vertebrateswithsignificant similaritiesbetweentheirimmune
systemandtheinnateimmunesystemofmammals [20]. Thereare
also goodcorrelationsbetweenresultsobtainedfrommiceinfec-
tion studiesandthoseobtainedinthegalleria [21]. Interestingly,
the atypical A.terreus isolate collectedinmonth46(post-azole
treatment)displayedincreasedvirulenceinthe G. mellonella infection
