Neurologic examination showed slowness of speech and mild loss of facial expression on both sides. The patient had difficulty initiating movements. Once seated, she did not move about much. Her posture was stooped and she walked with a small-stepped gait, with decreased arm swing. There was no muscular atrophy and no weakness. Muscle tone was increased in the arms, and “cogwheel rigidity” was present. There was a fine tremor in the fingers of the right hand (frequency of three to four times per second). The rest of the examination and the laboratory data were within normal limits.
