Use of SCDNT in diabetes self-management education served as a guide in identification,
planning and implementation. In the course of identification, a wide perspective was provided
by determining the existing problems and which problems may occur in the patient’s self-care
attitude. Her self-care behavior has provided a wide range in evaluating the basic conditioning
factors which influence the self-care and in the inspection of the environment. Nursing planning, has been guided by the self-care agency of the patient who is supposed to take care of her requisites and how the responsible person can help her by means of nursing systems. The
implementation has demonstrated what can be done to improve self-care behaviors and how
to address the subject (education, guide etc.). Therefore, it increased the patient’s involvement in her own care and brought positive changes to her health status.
