When I got my post-masters and then my DNP, I built on the
research I had done for patient safety and CPOE systems. When
we went electronic, suddenly, you could actually read the chart.
I don’t think anyone has ever done any studies on that (because
I don’t know how you would do it), but just the fact that you can
actually read the physician’s writing unambiguously – it has to
have saved lives. It’s hard to capture it and say definitively what
the numbers might be, but I have no doubt that it has. Then there
are things you can build into the system that you could never do
with a pen and paper system – things like allergy alerts, duplicate
checking, or interactions with drugs. Even just these things have
been huge in patient safety.
Then there are things you can put into a system (like templates)
so important steps aren’t forgotten. It might be a template related
to how a patient who comes in and appears to be septic should
be treated. If you can build that into the system and have the
information right there, you know within three hours you need to
get a blood culture done, an antibiotic administered, and a lactate
drawn. All these things are based on evidence and can be put in
as queues and reminders.
We have a nurse-driven protocol for catheters that allows a
nurse to remove them when it’s appropriate, based on the Centers
for Disease Control and Prevention (CDC) guidelines. We built that
into the system so that when the provider orders the initial catheter,
there is a place that reminds him or her to follow the protocol. It
tells the nurse, “Should the nurse follow protocol? Yes.” If no, you
have to say why not. Then there is the criterion from the CDC that
says things like “Patient is on strict intake and output, not clinically
stable, just had a urinary procedure done,” and so on. When the
physician says to follow the protocol, which 95% of the time they
do, then the providers never see an alert. Then it’s up to the nurses,
and they can take the catheter out when the patient is no longer
unstable. They will then see an alert every 24 hours if they’ve not
documented why it’s still in. If they’ve documented why it’s still in,
then they don’t see an alert either. When they do see an alert, they
are shown a link that goes directly to the flowsheet to document
why the catheter is still in. As a result, we have seen our catheterassociated
urinary tract infection (CAUTI) rates go down. In fact,
we had three hospitals in the last quarter of 2015 that reported no
CAUTIs. We were anywhere from 12–18 per month, and now it’s
gone down dramatically. We are sustaining that rate at a really
low number. We’ve had some months with zero incidences.
There are two sides of the coin, however. You can take
advantage of an EHR in ways you never could before to improve
patient safety. The other side of that is that your EHR can be the
cause of unintended consequences if you have it set up in way that’s
unsafe. An example is that of adjacency errors. Say a provider puts
all of his or her lists in alphabetical order, and you have to put an
order in for Mr. Smith. If you have five Smiths in a row, the wrong
patient can easily be chosen (and it happens a lot more often than
you’d think). In some systems, you’re allowed to have two patients
up at the same time on a tab. If you’re toggling back and forth, you
can get distracted and accidentally put an order in on the wrong
patient. We’re going down the path of having incident reporting
systems capture events related to health IT. I recently met with
our incident reporting vendor, and we’re looking at how we can
improve how we capture the incidences that have a relationship to
the EHR.
One of my goals as ANIA President was to get a position
paper written during my term. The paper I wrote is about how to
best address patient safety issues in organizations. What are the
things that an organization can put in place to make sure they have
a good patient safety health initiative? The important thing is that
there is an easy and clear way to document this. There needs to
be a clear place to document so that the nurse or doctor entering
the incident can do so easily. The efforts around patient safety are
incorporated with the global safety effort of the organization. The
Office of the National Coordinator has actually developed an
action and surveillance plan for monitoring health IT safety. It’s
really about continuing to learn, recording data in a better way.
