6. A critical comparison of the options available for control of T.
solium transmission
Table 1 summarises the author’s opinions about the relative value
and potentials of various options that are available to control
transmission of T. solium. Clear quantitative data are not available
for many aspects and hence the rankings provided represent the
author’s perceptions more than judgments supported by good
quality data.
6.1. Public education
Many of the options available to prevent T. solium transmission
and human neurocysticercosis are obvious from an understanding
of the life cycle of the parasite. Educating people in endemic areas
about the parasite and the diseases it causes, together with implementing
the behavioural changes necessary to interrupt the parasite’s
transmission, has great potential to reduce T. solium
transmission. These behavioural changes include the use of latrines
rather than open air defecation, washing hands after defecation
and corralling pigs so that they do not have access to human feces.
Behavioural changes concerning sanitation have enormous potential
benefits, well beyond their potential to prevent T. solium transmission
and this fact provides additional impetus to the adoption
of public education as a tool to achieve T. solium control.
Apart from the corralling of pigs, there are few if any potential
negative outcomes from public education as a means for control of
T. solium. With respect to the permanent corralling of pigs, this
would have at least two possible negative outcomes. Unless it
was simultaneously implemented with a cessation of open air defecation,
it would be likely to lead to an increased contamination of
the environment with human feces that otherwise would be consumed
by the pigs. Also, the situation where T. solium is most endemic
is amongst very poor subsistence farmers (Gilman et al.,
1999; Willingham and Engels, 2006). Allowing the pigs to roam
free and find their own food means that the farmers do not need
to provide them as much pig feed themselves. Permanent corralling
of the pigs would require that all food was supplied to the pigs
and this would impose additional expense on already very poor
farmers. This cost could potentially be counteracted by there being
a greater value placed on cysticercosis-free pigs (Gilman et al.,
1999; Willingham and Engels, 2006).
Several experimental interventions for T. solium have been performed
in which public education was undertaken, either as the
sole intervention or together with the treatment of tapeworm carriers.
Allan et al. (2002) described a trial undertaken in north-central
Gansu Province of China from 1978 involving both public
education and mass treatment of the human population with praziquantel.
After bi-annual ‘targeted’ treatment of taeniasis cases
both human taeniasis and porcine cysticercosis were reduced.
Keilbach et al. (1989) implemented a community education
program together with mass treatment of the human population
with praziquantel in a rural village in the Mexican state of Guerrero.
One year after the intervention, cysticercosis prevalence was
determined in pigs of less than 1 year old, by tongue palpation.
The prevalence was found to be almost twice as high (11%) after
the intervention as it had been previously (6.6%). It is unclear
whether the difference was statistically significant. The authors
comment on the difficulties they had obtaining the cooperation
of the community and the limitations this placed on the
intervention.
Pawlowski et al. (2005) described an investigation comparing
the effects of implementing either a 6 month intensive health-education-
only approach for T. solium control with health education
plus mass praziquantel treatment of the human population, in
two Mexican villages. The effects of the intervention were evaluated
6 months after the completion of the intervention. Details of
the effects seen in the village which received the education-only
intervention have been published by Sarti et al. (1997) but the effects
of the combined interventions do not appear to have been
published. No statistically significant differences were seen in
either the rate of human taeniasis when comparing the pre- and
post-intervention assessments in either village. However improvements
were detected concerning knowledge about the parasite and
there was a statistically significant reduction in porcine cysticercosis
detected by either tongue palpation or serology, after the
intervention.
Ngowi et al. (2008, 2009) undertook a study of the effectiveness
of a public education intervention to control T. solium transmission
in the Mbulu district of Tanzania. Following an intensive education
program, sentinel pigs were distributed to households and the
knowledge and practices of people were assessed by questionnaire.
Porcine cysticercosis in the sentinel pigs was assessed 1 year after
the program was undertaken, using a serological technique. Comparing
a control community and one where the education program
was undertaken, no statistically significant improvement was
evident in several critical measures relating to knowledge about
transmission of T. solium (e.g. how pigs get cysticercosis) although
