The IHC-WOW project, funded through a US Department of Health and Human Services Resources and Services Administration grant, brings IHC to community settings in the form of nurse practitioner house calls, group primary care visits, and tele-monitoring. These new approaches target high-risk Thresholds consumers who do not have a primary care provider or do not regularly access primary care. Over the past two year, the IHC-WOW project has not only increased access to primary care for individuals with psychiatric disabilities who require more intensive levels of mental health services available, it has presented opportunities for further integration between the partner organizations.
