DASS Q5 - 'things' rather than thing.
Relapse (title change to 'relapse risk questionnaire') Quick brief on what it is "This scale measures you risk of possible relapse and allows us to assist in providing further services or support if required" or something like that
Q3 and Q4 Just a Yes/No
Q5-7 I think maybe if we did a 5 point scale for these we would get a better idea. Not at all, Sometimes, Undecided, Often, Always.
DTCQ - All good, just remove asterix at the bottom - people may go ahead and complete several times, and we really only need the primary at this time.
As for the client name, will this be a consistent code that they will be using across all their assessments? I think that we can put the timing together for clients in terms of when they were there, no real necessity for that. Don't need counsellor details.
