Bactericidal drugs are preferable because of the
poor immune response within the CNS. As a result,
intravenous administration is required because
of the ability to provide high peak blood levels.
Intrathecal administration of antimicrobials has been
described, however there is little evidence of efficacy.
Bacteriostatic drugs may be useful in some cases of
brain abscess and spinal abscess, particularly drugs
such as chloramphenicol that have other desirable
properties.
