The proper procedure to investigate such cases is to form a committee and not for the direct supervisor to investigate and report. GM - you need to have an SOP for to carry out the investigation of such matters as well as all employee disciplinary matters.
Secondly, it was reported that one of the incidents was due to staff's ignorance of the procedure (i.e. SOP). This is no good as it means that either HRB does not have the SOPs or our staff are not familiar with them.
I have mentioned many times that management MUST have SOPs to address ALL the key risks areas (I had provided these to management long time ago.) In addition, ALL employees must be familiar with ALL the SOPs.
For these two incidents, I will accept FOM's explanations. But, in the future, the above MUST be adhered to.
