When visit the doctor at non-designated hospitals (not in BUPA hospital list) or have the dental care (with any hospital or clinic), staff members must submit the following supporting documents for claim reimbursement to Human Resources Department.
1. Original receipt with your reference number written down in the receipt.
2. Doctor Certificates with diagnosis
All claims must be submitted within 80 days after treatment (outpatient) or after leaving the hospital (in-patient).
The late submission may cause the unclaimable payment if no justified reasons explain.
