The most important test for assessing MCL injuries is the valgus stress test, also known as the elbow abduction stress test. It involves applying pressure to the inner aspect of the elbow joint, assessing for symptoms and comparing the laxity or instability against valgus forces. It is recommended to apply valgus stress to the elbow at a flexion angle of 20-30 degrees, then assess the endpoint quality. If there is no defined endpoint, a positive test is indicated when there is an opening gap of more than 3 mm or when the patient experiences pain.14,48 Injury to the MCL may be confirmed by positive results from valgus stress testing or the milking maneuver.41,43
