Assessment includes the teacher observing and documenting what they see when the child works. Before observing and documenting a child's assessment, the teacher must have outcomes for each set of work a child is doing. Outcomes are based off of state or national standards and must be measurable. Once there are measurable outcomes written, the teacher can observe the child. If the class is large, a teacher might assess students while doing group work. If the class is small, and there are teacher aids, a teacher can assess one child at a time. In order to be filed properly in the student's file, the teacher who completed the assessment must formally record all of his or her observations of the student. The documents must be dated and there must also be with the observations a brief statement explaining what work the student did and any additional details the teacher feels should be included in the paperwork.
