• receives medical treatment;
• loses consciousness;
• is diagnosed with a significant injury or illness (e.g., a fractured or cracked bone, punctured ear drum, cancer or other chronic irreversible disease);
• is off work for one day or more, not including the day on which the injury occurred;
• is allowed to return to work but is not allowed to do specific tasks or bodily motions due to doctor's orders;
• is allowed to return to work but to a different job because the doctor's order not to do specific tasks or bodily motions prevents the employee from doing his normal job;
• dies from a work related incident.
