Check i) whether there have been any Lost Workday incidents within the past year, and ii) if they have been reported under the following procedures:
a) The supervisor must immediately call in the incident report to EESH department and report to head of BU;
b) When a Lost Workday incident arises, the head of department must, within twelve hours of the incident, report to head of BU and Company Presidentand notify head of EESH department;
c) The head of BU shall organize an investigation team to identify fundamental cause of incident, and the head of department shall draw up an incident report to be reviewed by head of EESH department and then reported to Company President; and
d) The head of department and EESH department must establish preventive measures for such incidents and inform all members.
