Please find opportunity improvement from Chevron VIP Visit comment in detail below, Khun Bob want us to close out gap that could happen on our rig:
• Opportunity to discuss during each job planning meeting, “how could anything conceivable drop during this job & what can we do about it?”
• Provision of boot covers upon arrival to assure can cover boots inside when arriving with steel toed shoes on
• While seen at some locations, opportunities to increase participation in pre-job meetings by asking job specific questions, i.e. asking:
o How does the tenet of the day relate to this job
o Each team member how they could get hurt specifically on the task to be done
o Each team member what hazards they have associated with their specific task
o How could a drops event occur related to their specific task
• More opportunities to use debrief boards so that team members can visualize the idea from one team member & stimulate ideas for others
• More opportunities to have a process to capture ideas during execute so that they are not forgotten when debrief occurs
• Starting every meeting, including Pre-Job meetings & debrief meetings with the Tenet Of The day
• Asking: tell me what you understand vs asking do you understand. Open ended questions only.
• Assigning someone on the team during each pre-job meeting to be responsible to keep an eye on pages 2 & 3 of the Pocket book to assure nothing is missed.
• Opportunities to have observers who come to observe pre-job meetings use the observer’s check list on Page 20 of the IFO pocket book
• There is an opportunity to use check lists to validate & verify safeguards are in place & effective prior to the start of doing work
• May help to determine if new arrivals have a prompt card & if not to be sure to issue one for use.
• More rigor around using the Pre-Job agenda check list on Page 2 & 3 of the Pocket book to be sure nothing is missed to have the same level of discipline as pilots do to assure a high reliability organization 100% of the time. Everyone agreed we wouldn’t feel comfortable with a pilot not using a check list and achieving 99.9% reliability. May be an opportunity to assign someone at each pre-job meeting to be the keeper of the pre-job check list & to assure everything is discussed in strict order.
• Not all chin straps on board are 3 point, although it is listed as a Safety Management System requirement during the arrival induction.
