1. Please review the list and consider what you would like the committee to do first, mark those items 5 then on a descending scale mark other items 4, 3, 2 and 1. One being the lowest priority. If you think the item will not benefit the residents mark it 0.
2. If there are any items that you would like to see included in the list please add them to the proposed maintenance survey in the space provided at the bottom of the form.
