According to the experience at the Burdenko Neurosurgical
Institute and the published data,6,16,17,19 microsurgery
is the primary method of treatment for large and giant partially
thrombosed MCA aneurysms.
To determine the treatment strategy for cases of partially
thrombosed saccular MCA aneurysms, one should
first focus on the absence or presence of a thrombus in
the neck.11,17,21 According to this criterion, we identified 2
types of MCA aneurysms. For the first type, we performed
conventional neck clipping, and for the second, we performed
neck clipping after preliminary thrombectomy.
