The Statement of Ownership and Authority is a legal affirmation required for Apple to remove Activation Lock from a device owned by a business or Institution. Although a device may be owned by a business or Institution, an Apple ID can belong to an individual. The Statement of Ownership and Authority provides Apple the permission to remove the Apple ID from the affected device.
Additionally, we need a product identifier on your proof of purchase for it to be considered valid. Please add the serial number, IMEI number or MEID number of your device on your proof of purchase. Writing this on the document is acceptable.
In addition, We need a clear photo of your product’s IMEI or Serial Number, located on the device itself or the SIM card tray. For models that do not include a SIM card tray, please include a screenshot of the serial number in iTunes, iTunes Diagnostics, or System Profiler. A photo of the IMEI number from the display of the device would not be considered valid.
