6. Schedule Control / Cost Control: Describe the company’s approach to managing the project
schedule and project cost. What tools are used to monitor the schedule and costs? How often
are tools updated? How are updates communicated to team members? Describe the company’s
track record in controlling project schedules and costs.
7. Safety Program / Record: Safety is a critical component for a successful project. Describe the
company’s safety program and explain how safety will be managed for this project. Provide a
copy of your company’s Occupational Health and Safety Policy (OSHA form 300).
8. Legal History: Provide a five (5) year history of lawsuits and litigation including any current
pending litigation. Describe the nature of each legal action and its result. Clearly identify if the
company has any history of litigation with the Phoenix Zoo.
9. Conflict of Interest: The Contractor is required to disclose any actual, apparent or potential
conflict of interest that comes to its attention in preparing its application, during the review of the
application or during the term of any subsequent agreement with the Phoenix Zoo. The Contractor
shall accept any determination by the Phoenix Zoo as to whether the Contractor has a conflict of
interest and the consequences of having such a conflict of interest, including disqualifying any
application, requiring the Contractor to withdraw from any bidding or terminating any agreement
awarded to the Contractor. Conflicts of Interest are defined as “any situation in which an individual
or corporation (either private or governmental) is in a position to exploit a professional or official in
some capacity for their personal or corporate benefit.”
10. Qualification Statement: Complete the attached General Contractor Qualification Statement
and submit it with any other requested materials.
