The PEP was launched in 2010 as a tertiary-wide primary
care service component across the Hong Kong Hospital
Authority with the purpose of providing quality chronic disease
management to enhance primary care services. In the
first evaluation cycle, from 1 March 2010 to 30 September
2010, two non-governmental organizations (NGOs), highly
experienced in providing community medical education services,
were invited to participate in the programme. From
August 2011 onwards, four NGOs were invited to deliver
the PEP, offering full coverage of services across all district
clusters in Hong Kong. A detailed description of the PEP
setting, mode of education delivery and results from the first
evaluation has been published previously [13]. The main
function of the PEP is to deliver sessions on disease-specific
knowledge, self-management skills, self-efficacy and lifestyle
modification to patients with T2DM. The present analysis
summarizes the second set of evaluation data derived from the
quality-of-care evaluation of the PEP provided by four NGOs
with subsidies. This analysis included patients who attended
at least one session of the PEP between 1 March 2010 and
30 June 2012.
